 Post School Programs

​After completing Year 12 many students with disabilities in the Independent Sector will go directly to employment, vocational education and training or higher education.

Some students with disabilities, however, need support to transition to work or to participate in community life. The Ageing Disability and Home Care Department (ADHC) provides two Post School Programs for eligible young people with a disability, the Transition to Work Program and the Community Participation Program. These programs support young people with moderate to high support needs to develop the skills they need to work towards their goals, increase their independence, and participate as active and valued members of the community. Students in the independent sector who wish to make application for these programs are assessed by one of the AIS consultants who specialise in disability. Schools will be asked to identify these students during Term 1, 2011. The assessments will be carried out after ADHC runs training sessions for consultants in late March. After this time a consultant will contact the school to negotiate assessment times. The assessments are used to determine eligibility for the programs and ADHC will communicate the results of the assessment directly to families. Families are also provided with information regarding local program opportunities for the beginning of 2012.
