Post School Planning for Year 12 School Leavers with Disability
School-based transition planning is important to assist the student with disability, their family and school gather information on the student’s post school goals, pathways and support needs
There is an ongoing need for teachers to support all Year 12 students with disability to complete their personalised transition planning during terms 2 and 3, or earlier. For many students, this will build on previous or existing planning.
Representatives from the three sectors have worked collaboratively to develop the following resources designed to support young people, their families and schools with the transition planning process.
